Consumer Health Data Privacy Notice
Effective date: October 6, 2026 · Last updated: October 6, 2026
This notice explains how Rejuvyx Health, LLC ("Rejuvyx," "we," or "us") collects, uses, and shares consumer health data as defined by Washington's My Health My Data Act and similar state laws, including Nevada's and Connecticut's consumer health data laws. It supplements our Website Privacy Notice. If this notice conflicts with our Website Privacy Notice regarding consumer health data, this notice applies.
This notice is separate from, and does not replace, the Notice of Privacy Practices ("NPP") applicable to medical care provided by an independent healthcare provider. When information constitutes Protected Health Information ("PHI") under HIPAA, the applicable provider's NPP governs the provider's use and disclosure of that information. Where Rejuvyx acts as a business associate, Rejuvyx handles PHI in accordance with the HIPAA requirements applicable to its role and its contractual obligations to the applicable healthcare provider.
